What is Community Care 1st?
Community Care 1st (CC1) is a Limited Benefit ERISA Plan (Employee Retirement Income Security Act of 1974) that provides Plan members with unlimited access to Urgent Care services at four (4) locations in the Northeast Florida area. CC1 medical providers have extensive experience in providing the highest quality of professional service to Plan members in an Urgent Care Facility.
Who is eligible to participate?
CC1 is available to employers who create a plan under Section 105(h) of its IRS code for the benefit of their employees. Each employer defines their eligible class of employees for the program subject to the non-discrimination rules of Section 105(h). Employers must enroll at least ten (10) employees or pay the minimum monthly fee of $390 in order to qualify for the program.
Who pays for the CC1 program?
The employer, the employee, or a combination of contributions from both may be used to cover the costs of the Plan.
How much do Plan members pay when they see a medical provider?
Plan members pay a $10 copayment for a visit to one of the CC1 participating providers. If generic prescriptions are dispensed at the time of service, then there will be an additional charge of $4 per prescription. Diagnostic tests may also be available if medically necessary and prescribed by your CC1 provider. Additional co-payments will apply to these tests.
What about hospitalization and catastrophic coverage?
CC1 is a “limited benefit” ERISA Plan only. Services are limited to those available at our four (4) participating Urgent Care centers. The CC1 Plan does not provide any coverage for care delivered outside of our participating medical providers. There is no coverage for hospitalization or any form of catastrophic procedures. Urgent Care services are for acute medical care and not for long-term care needs. CC1 does not offer primary care.
May an Employer provide the CC1 program to their Employees if they also provide a Health Savings Account (HSA)?
No, the HSA guidelines prohibit an employee from maintaining any types of coverage for services under their High Deductible Plan unless it is for preventative care.
May an Employee contribute towards their CC1 coverage Fees on a “pre-tax” basis?
If their Employer provides a Section 125 POP Plan, the Employee may pay for their CC1 contribution on a pre- tax basis.
What are the types of medical services that are covered under the CC1 program?

Is there a limit on the number of times a member may visit a CC1 provider during the Plan Year?
No, there is no limit on visits for covered members.
May an Employer offer CC1 to part-time Employees?
Yes, the Employer may define the Eligible Class of Employees for the CC1. They must meet the Non-Discrimination Testing Guidelines or the Section 105(h) in order to qualify for tax free status for all participants.
How do I qualify for Diagnostic Testing such as CT Scans, MRIs, etc.?
Diagnostic tests must be considered medically necessary and be prescribed by your CC1 provider. With prior approval and a $40 copayment per test, the diagnostic tests will be performed at an affiliated facility.
Does Urgent Care treat everything?
No, the Urgent Care center does not treat long-term chronic care. Physicians, Medical Centers - Jax, Inc. is here to treat patients without life threatening emergencies.
