What is Community Care 1st?

Community Care 1st (CC1) is a “Limited Benefit ERISA Plan” sponsored by employers and administered by Medcom. The coverage is only available for care provided at one of the four (4) Physicians’ Medical Centers – Jax, Inc. (PMC) in Duval and Clay County.

Urgent care is generally defined as the delivery of ambulatory care in a facility dedicated to medical care outside of a hospital emergency department and usually on an unscheduled basis. We provide a full staff of seasoned medical care specialists with significant experience in acute and emergency care. Our services are comprehensive in nature and include lab work and diagnostic testing in addition to physical examinations and counseling from our fully licensed and credentialed caregivers. Our extended work hour’s enable our patients to enjoy the convenience of better access to care in a professional environment with limited to no wait time. We are open 365 days a year from 8:00 a.m. - 8:30 p.m. with the exception of the Northside location, which closes at 8:00 p.m.

The Plan is designed to provide comprehensive Urgent Care services, including the following types of urgent medical care injuries/illnesses.

Asthma, Shortness of Breath
Sprains/strains
Fractures
Urinary/bladder infections
Respiratory infections
Physicals
Vaginal/penile infections
Other illnesses/injuries

Cold/flu symptoms
Cuts/scrapes/bruises
Ear infections
Migraines
Motor vehicle injuries*
Workers’ Compensation injuries*
Fever
Animal bites
Skin infections
School & Sports physicals
Seasonal Flu vaccine (upon availability, and only available after 6 months of enrollment with appointment)

*All motor vehicle injuries and Workers’ Compensation accidents will be filed with the motor vehicle insurance or Workers’ Compensation insurance.

Urgent Care treats patients without life threatening emergencies. If you have a life threatening emergency you need to call 911 or go to the nearest emergency room. Patients often require additional medical care after an Urgent Care visit. PMC providers will advise the patient of where and what type of additional medical treatment is necessary. In many cases, the follow up care will not be covered under the Community Care 1st (CC1) Plan.
