Community Care 1st offers unlimited access at four (4) high quality health care centers in Duval and Clay County. Monthly coverage fees start as low as $39.00 for single coverage, $70.00 for two family members and $95.00 for a full family. Once enrolled, you and your family may visit any one of our facilities from 8:00 a.m. – 8:30 p.m. any day of the week on a walk-in basis. Member access fees at our Centers are only $10.00 per office visit. An additional $4.00 is applied for each of our in-house generic prescriptions filled on-site from our CC1 pharmacy list. Other fees may apply for advanced diagnostic testing, if available.
